What malpractice insurance documents do I need to provide during onboarding or before my collaboration agreement is sent?
You need to provide proof of current medical malpractice insurance, such as a certificate of insurance (COI) or other documentation showing active coverage. You also need to complete the required agreement information fields and submit your NPDB self-query report before the collaboration agreement can be sent through DocuSign. A DEA license is only required if it applies to your role, such as if the clinician will be prescribing controlled substances.
