When is the NPDB Self-Query due, and how often is it required?

For clinicians, the NPDB Self-Query is requested when starting a collaboration. For physicians, the NPDB Self-Query and disclosures are requested before confirming the first collaboration on Single Aim. After that, physicians are generally asked about once a year to provide updated NPDB Self-Queries and disclosures.
